Job description

This position will work 830-5 or 9-530, with weekend and holiday rotation.

The Intake Nurse is integral in the coordination and execution of the admission process to Hospice services. Demonstrates excellent customer service skills with patients, families, customers, and co-workers. Must be resourceful in overcoming hospice barriers with patients and caregivers. Responsible for obtaining referral orders and patient records.

Unrestricted LPN or RN license.ย  Minimum of 2 years of nursing experience in home health, or healthcare facility.. Must be able to deal with human suffering while maintaining professional boundaries.ย ย Required to be flexible and change schedule at a moment's notice to meet patient needs. Must have excellent clinical skills and be able to identify/solve problems, consult with the physician, and execute orders to meet patient needs.

  14. Obtain medical records as needed based on requests from admission nurses, liaisons, and Hospice
  15. Enter and document Care Navigation (Care Nav) referrals, including task notes, contact information, and service
  16. Enter Palliative Care referrals, track consult times, ensure consult notes are signed, email billing info, and monitor patient discharges.
  17. Support the entry and update of new referral accounts and physicians in Referral Manager (RM) for Arkansas and
