Registered Nurse, Intake

Join PACE CNY as a Registered Nurse, Intake!

The Registered Nurse, Intake assists with the marketing of PACE CNY to the referral community, follows up on all referrals to the program, coordinates the intake process for potential new enrollees, strives to meet the enrollment goals of PACE CNY, and assists in orienting all PACE CNY employees and contracted providers about PACE CNY and prohibited marketing practices.

Schedule: Monday-Friday: 08:00 AM- 04:30 PM

Salary: $80,101-$88,233.60 Annually

No Holidays or Weekends!

Responsibilities

Intake

  • RN meets with potential participants and their families to discuss the philosophy of PACE CNY, eligibility criteria, PACE services and how PACE is funded.
  • Obtains signed consents/authorization needed to start the PACE assessment/enrollment process.
  • Works with Finance Dept to ensure payment source.
  • Coordinates a visit day to PACE Center often with PACE transportation.
  • Completes an Intake based on potential participant's ADL/IADL needs. (Enters this information into the electronic medical record.)
  • Completes a nursing assessment to determine medical eligibility (UAS) in The Health Commerce System.
  • Attends scheduled Intake & Assessment meetings to provide information on potential participant to the interdisciplinary team to come up with an initial care plan.
  • Other Intake duties as needed.
  • Marketing
  • Assists in marketing the PACE CNY program.
  • Provides small in-services and informal presentations to potential referral sources, community groups, Loretto employees, etc.
  • Follows DOH and CMS Marketing Guidelines and adheres to Prohibited Marketing Practices.
  • Assists with other marketing duties as assigned.
Qualifications
  • Degree from an accredited school of nursing. BSN or BS in related field preferred.
  • Current NYS license and registration as an RN in good standing required.
  • Some background in marketing communications or sales preferred.
  • Experience in the delivery and marketing of elder services preferred.
  • Must possess a valid driver's license without restrictions and a reliable, insured vehicle.
  • Current knowledge of best practice nursing standards and care.
  • Proven patient assessment skills.
  • Self-directed and able to maintain schedule.
  • Ability to exercise good judgment and maintain confidentiality.
  • Proven organizational, time management, and multi-tasking skills
  • Strong verbal and written communication skills.
  • Strong customer service and interpersonal skills.
  • Basic computer skills in MS Office preferred.

An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.
