Job Type: Full-Time/Part-Time
Schedule: Baylor (Weekend Schedule)

Senior Care Solutions is seeking a compassionate, detail-oriented Baylor Intake Coordinator to join our growing home health team. In this role, you'll help ensure patients receive timely, high-quality care by coordinating referrals, processing admissions, and collaborating with our clinical teams throughout the weekend.

Responsibilities
  • Process and coordinate new home health referrals and admissions.
  • Verify patient information and ensure all required documentation is complete.
  • Communicate with physicians, hospitals, case managers, and referral sources.
  • Coordinate with clinical staff to facilitate timely Start of Care visits.
  • Maintain accurate patient records in the EMR system.
  • Monitor referral status and ensure compliance with Medicare and agency guidelines.
  • Provide exceptional customer service to patients, families, and referral partners.
  • Collaborate with the weekday intake team to ensure seamless continuity of care.
  • Previous intake, scheduling, or healthcare administrative experience.
  • 1+ years of Home health/Medicare experience.
  • Strong organizational and multitasking skills.
  • Excellent verbal and written communication.
  • Proficiency with Microsoft Office; EMR experience preferred (WellSky/Kinnser experience is a plus).
  • Ability to work independently while managing multiple priorities.
  • Detail-oriented with strong problem-solving skills.
