Clinical Intake Specialist-Home Health & Hospice

Primary responsibilities will be:

  • Providing an outstanding admissions experience for our patients
  • Communicating effectively with patients, family members, Case Managers, healthcare providers and co-workers
  • Providing education to patients and family members regarding their home healthcare experience
  • Reviewing patient charts and ensuring information is clearly communicated to the patients care team
  • Serving as a point of contact throughout the admissions process
  • Ensuring coordination of care with other team members

Must-have’s to be considered for this position:

  • 1 year of Intake experience (Home Health/Hospice)
  • Ability to Multi-task with High volume referrals
  • Insurance Verification and multi hospital portals (experience is a must)
  • Proficient on all Microsoft Office software (Word, Excel, PowerPoint, Outlook)
  • LVN is a plus but not mandatory
  • Experience with HCHB EMR
