Job Description
Safe Life Home Health is seeking an office based Clinical Case Manager to support referral intake, case triage, coordination of care, and communication with patients, families, referral sources, and the clinical team. This role is ideal for an RN, who is organized, responsive, and comfortable managing a high volume of incoming cases in a fast paced home health setting.

We now serve patients through two branches:

  • Lombard branch with a 4.5 stars Medicare Quality Rating

  • Chicago branch with a 5 stars Medicare Quality Rating

The Clinical Case Manager will play a key role in reviewing incoming referrals, triaging cases appropriately, helping determine eligibility and service needs, coordinating with intake and clinical staff, and ensuring smooth case flow from referral through admission readiness.

Pay Range
$35-$40 per hour

Key Responsibilities

  • Provides oversight of all patient care services and personnel including but not limited to making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring patient needs are continually assessed, and assuring the development, implementation, and updates to the individualized patient plan of care

  • Serve as the first line for resolution of concerns and issues for the staff

  • Plans, directs, and participates in delivery of home health services to provide diagnosis-specific patient care, including coordination of services with providers, vendors, facilities.

  • Coordinate with hospitals, physicians, discharge planners, visiting clinicians, patient/caregiver, and internal teams for patient care coordination

  • Provide employees with continuous training and education and perform employee reviews, exits, counseling, and meetings

  • Communicate regularly with support staff to assess current needs, determine level of accountability, and provide supportive discussion related to areas of responsibility and job duties

  • Develop staff to meet quality, safety, and competency requirements by assisting with training and professional development of new and current staff, and annual joint visits

  • Communicate with leadership regarding continuous quality improvement, problem solving, and other issues related to the overall effectiveness of the Home Health program

  • Collaborate with the Director of Nursing / Clinical, Intake team, QA team, and branch staff to support efficient case management workflow

  • Develop staff to meet quality, safety, and competency requirements by assisting with training and professional development of new and current staff, annual joint visits, and setting up vacation coverage

  • Help prioritize urgent and high acuity cases for timely response

  • Ensure compliance with company policies and applicable home health regulations

  • Support both Lombard and Chicago branch operations as needed

Qualifications

  • Active RN license in good standing

  • Prior experience in home health, intake, referral management, case coordination, or clinical triage preferred

  • Strong understanding of home health services, documentation, and care coordination processes

  • Excellent communication, organizational, and multitasking skills

  • Ability to work efficiently in an office-based, team-oriented environment

  • Strong attention to detail and ability to manage multiple cases simultaneously

  • Experience with Kinnser is a plus

Preferred Skills

  • Knowledge of Medicare and other payer guidelines related to home health admissions

  • Experience communicating with physicians, hospitals, rehab centers, and referral partners

  • Ability to identify priority cases and escalate appropriately

  • Comfortable using EMR systems and standard office software
