Position Overview: The Intake Case Manager (CM) - Bilingual has the responsibility for developing, authorizing, coordinating, and implementing community-based services to older adults and people with disabilities.
Position Responsibilities: Receive assignments from Home Care Triage for potential consumers. Conduct Initial Assessment to determine eligibility for GLSS programs. Develop appropriate service plan based EOEA-mandated assessment tool, and process all paperwork/computer work. Make referrals for Home Care and/or other community services according to the Home Care referral protocol. Coordinate with GLSS Referral Specialist and service providers to ensure timely and non-duplicative service delivery. Contribute to an Intake Team identity and role based on consumer engagement, outreach, motivational interviewing, multi-functional assessment, and consumer driven care.
Qualifications:
  • Bachelor's Degree in human services, social work, or related field preferred. Minimum of one (1) year experience in the human services field in lieu of Bachelor's Degree with the ability to obtain a wavier from Executive Office of Aging & Independence (AGE)
  • Bilingual in Spanish, Russian, or Khmer preferred
  • Expert time management with the ability to adjust to changing priorities
  • Strong desire to help others and advocate for those needing services and assistance
  • Excellent communication skills with the ability to establish positive and professional relationships with team members, providers, consumers, and their support system
  • Proficient computer skills
  • Strong documentation skills with the ability to complete documentation that is timely, accurate, and comprehensive
  • The ability to appropriately handle confidential and personal information, meeting the Agency standards for HIPAA compliance and data security
  • Demonstrates a strong work ethic, dependability, and commitment to consistently meeting responsibilities and deadlines
  • Valid Drivers License and Reliable transportation with the ability to conduct home visits throughout the GLSS service area

Hours Full Time 40 hours per week (Monday-Friday) Full Time 40 hours per week (Monday-Friday). Fully In office for the first ninety (90) days of the initial probationary period. Upon successful completion of the probationary period, will require a minimum of one (1) day per week in-office presence with other times spent on community-based assignments and home visits as needed.
Pay Range: $22.50-$24.50
