Job description

Job Summary:
The Intake Nurse - RN is responsible for assessing potential new members for eligibility for the program and for explaining the program to them and their families. Responsible for knowing required outreach activities within the community and the assessment of eligibility and safety of potential members. Collaborates with other members of the Intake/Outreach team and the clinical team to assure the appropriate provision of services to new members. Communicates with potential members, family, referral sources and physicians to assure a seamless transition to managed long term care.
Essential Functions:

  • Conducts outreach in the community as needed
  • Performs the initial assessment using the UAS (Uniform Assessment System) tool.
  • Evaluates potential members for eligibility and safety.
  • Determines the care needs of the potential member and formulates the initial plan of care.
  • Communicates with the clinical team to ensure seamless delivery of service.
  • Completes all enrollment documentation is a timely manner.
  • Serves as a community resource for intake issues.
  • Demonstrates the ability to integrate age specific requirements with individual developmental, mental, social and cultural needs when performing procedures for geriatric members.
  • Perform any other job related duties as requested.


Education and Experience:

  • Associates degree in Nursing required
  • Bachelor's degree preferred
  • One (1) year experience working in an acute care setting required
  • Home care and MLTC-intake experience preferred

Competencies, Knowledge and Skills:

  • Ability to define problems, collect data, establish facts, and draw conclusions
  • Ability to speak effectively before groups or customers or employees of organization
  • Must be able to read, write, and speak English
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals
  • Demonstrates the ability to utilize the equipment necessary to perform their job duties
  • Convey a positive attitude by being friendly, kind and interested in helping others
  • Create a home-like, healing environment by being courteous and respectful to all
  • Knowledge of Medicare and Medicaid regulations

Licensure and Certification:

  • Current Registered Nurse license in the state of New York required
  • Must have valid driver's license, vehicle and verifiable insurance. Employment in this position is conditional pending successful clearance of a driver's license record check. If the driver's license record results are unacceptable, the offer will be withdrawn or, if employee has started employment in position, employment in this position will be terminated

Working Conditions:

  • This is a mobile position, meaning that regular travel to different work locations, including homes, offices or other public settings, is essential.
    Will be exposed to weather conditions typical of the location and may be required to stand and/or sit for long periods of time.
    Must reside in the same territory they are assigned to work in; exceptions may be considered, due to business need
    May be required to travel greater than 50% of time to perform work duties.
    Required to use general office equipment, such as a telephone, photocopier, fax machine, and computer
    Flexible hours, including possible evenings and/or weekends as needed to serve the needs of our members
  • Over 50% (Mobile) Routine travel required

Compensation Range:

$100,000 - $135,000

In addition to base compensation, you may qualify for a bonus tied to company and individual performance.
