Summary:

Provides direction and organizational management of the home care intake department including referral management, referral portals, physician group referrals, SNF/ALF discharge planning meetings, and the transition of patients from the acute setting to home. Responsible for the educational, orientation, and competency programs of the agency. Per diem nursing staff report directly to this position for recruitment, hiring, and performance evaluation. Supports the other nursing clinical managers in the department daily operations as needed.

Responsibilities:

1. Manages the planning, implementation, data collection, and evaluation of the referral management/intake process; and directs the transition of patients from hospital to home with home care services from hospitals, skilled facilities, assisted living facilities, and physician practices. 20%

2. Participates in the SNF discharge planning meetings and patient conferences including onsite SNF meetings. 5%

3. Recruits, hires, orients, and conducts ongoing performance evaluations of the home care intake staff and the per diem nursing staff. 15%

4. Works with the leadership of the network hospitals� care coordination departments, quality departments, and physician practice care navigators to manage the at-risk patient populations. 10%

5. Develops, conducts, and evaluates educational programs that meet the needs of the home care staff; and provides ongoing coaching to staff with the goal of improving critical thinking and technical skills. 15%

6. Responsible for planning and conducting the nursing orientation program, initial and annual competency programs, and ongoing educational needs of the home care nursing staff to ensure that the agency meets all Federal, State, and Joint Commission standards of care and maintains high quality value-based purchasing metric scores and Care Compare star ratings. 15%

7. Performs educational and supervisory co-visits with nursing staff to patient homes as part of the orientation, competency testing, and ongoing nursing staff educational needs. 10%

8. Participates in the rotation of the weekend agency supervisory coverage in the delivery of care on weekends and holidays. 5%

9. Participates in program planning, budgeting and evaluation activities related to the clinical services of the agency; including participation in the agency Quality Assurance Performance Improvement Plan (QAPI). 5%

10. Performs other duties as assigned.

Education Skills Experience:

Education: Bachelor�s degree in nursing and a minimum of three (3) years of clinical experience in nursing, at least one (1) of which was in home health or community health program which included care of the sick at home, or an Associate Degree in Nursing with 5+ years of experience, one of which was in home health or community health that included care of the sick,�and completion of 6 credits received in Community Health Nursing Theory, or 6 credits in Health Care Management from an accredited college or university program or school of nursing. The 6-credit course must be completed within 18 months of hire.

Credentials: Active State of Connecticut RN license; current BLS certification
