Building Location:East Range HospiceDepartment:2874010 HOSPICE - EAST RANGEJob Description:Provides social work services and discharge planning to patients and families in the healthcare setting to meet individual health and psychosocial needs in the promotion of wellness and cost-effective outcomes. This position also transitions patients to alternate levels of care and provides crisis intervention as needed.Education Qualifications:
