At St. Croix Hospice, the Social Worker plays a vital role on the patient care team, offering compassionate emotional and practical support to patients and their families in accordance with each individual's Plan of Care. This position involves meeting with patients and their loved ones, working collaboratively with the clinical team, and helping families access the appropriate community resources and services they need.

Social Worker Requirements & Core Responsibilities:
  • Bachelor's degree in social work from an institution accredited by the Council on Social Work Education.
  • Current Social Work licensed in the state(s) of practice
  • Four (4) licensure levels:
    • Licensed Social Work (LSW)
    • Licensed Graduate Social Worker (LGSW)
    • Licensed Independent Social Worker (LISW)
    • Licensed Independent Clinical Social Worker (LICSW)
  • Ability to pass background, consumer report, and drug test.
  • Conduct psychosocial assessments, identify safety or support needs, and provide resources, referrals, and care planning support for patients and families.
  • Provide counseling, crisis intervention, and bereavement support, including follow up for at risk individuals and coordination of bereavement activities.
  • Collaborate with the interdisciplinary team, maintain accurate documentation, support cultural and end of life needs, liaise with community agencies, and ensure HIPAA compliance.
  • Must have reliable transportation, valid driver's license, and proof of auto insurance

Social Worker Benefits & Schedule:
  • Schedule: M-F 8:00am to 5:00pm, Full-time position.

The starting salary range for this role is $54-$85K annual base salary. An employee's actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets. The total compensation package for this position may also include the opportunity to participate in employee benefits including our medical/dental/vision/pet insurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to applicable plan terms, and/or eligibility for other compensation.
