Hospice Social Worker
Location: Leawood, KS
Job Type: Hourly | Non-Exempt

About Us
At Legacy Hospice, we're committed to caring for patients and families with compassion and dignity. We are seeking a skilled and empathetic Social Worker (SW) to join our team. In this vital role, you'll help patients and families cope with the emotional, social, and practical challenges that come with a terminal diagnosis�bringing comfort and clarity during their most vulnerable moments.
What You'll Do
  • Conduct psychosocial assessments of patients and caregivers
  • Provide counseling to patients and families regarding emotional stress, coping, grief, and advance care planning
  • Assist with advance directives, funeral arrangements, and navigating legal/financial decisions
  • Identify and coordinate with community resources, agencies, and support organizations
  • Educate staff and families about cultural and psychosocial needs
  • Document patient conditions, interventions, and psychosocial support provided
  • Serve as a liaison between families and care teams to ensure coordinated, holistic care
  • Participate in interdisciplinary team meetings to develop individualized care plans
  • Provide crisis intervention and support for safety concerns (neglect, abuse, self-harm)
  • Participate in quality improvement initiatives and continuing education
  • Participate in on-call support as needed

Qualifications
  • Bachelor's degree in Social Work (BSW) required
  • Master's degree (MSW) or LCSW strongly preferred
  • At least 1 year of experience in social work, preferably in hospice, home health, or a community setting
  • Knowledge of end-of-life care, grief support, and cultural sensitivity
  • Strong written and verbal communication skills
  • Self-motivated, organized, and able to work independently
  • Current Basic Life Support (CPR) certification
  • Valid driver's license, auto insurance, and reliable vehicle required
