This position offers a remote work setting, allowing for flexibility and autonomy in managing your schedule. You will be part of a collaborative multidisciplinary team focused on delivering comprehensive care. As an LCSW at Clear Sky, you will have the following responsibilities:

• Conduct detailed bio-psychosocial evaluations to identify and address barriers to treatment, enhancing patient health status.
• Develop individualized treatment plans utilizing evidence-based treatment modalities and both individual and group therapies.
• Collaborate closely with medical and support staff to coordinate care, ensuring comprehensive support for patients and their families.
• Document patient interactions and social work activities in medical records according to department standards, focusing on maintaining accurate and concise progress notes.
• Provide education to patients and families on medical and social work recommendations, fostering understanding and compliance for optimal functioning.
• Monitor and assess patient and family advocacy needs, coordinating community resources to enhance social support.
• Attend relevant training sessions to continually develop expertise in areas pertinent to your clinical responsibilities.
• Uphold professional standards by adhering to the NASW Code of Ethics and internal departmental policies.

This position primarily focuses on serving patients within a hospice setting, where compassion and understanding are paramount. You will work closely with individuals facing life-limiting illnesses, their families, and caregivers, providing emotional support and assisting them through a challenging time. Your skills will help in addressing the psychosocial issues these patients encounter, enabling them to achieve a higher level of psychosocial functioning.

The ideal candidate will possess:

• A Master’s Degree in Social Work from an accredited institution and an active LCSW license issued by the State Board of Social Work Licensure.
• Demonstrated knowledge of caring for patients across various age groups, considering human development stages and cultural factors.
• Strong analytical and therapy skills necessary for evaluating patients, developing treatment plans, implementing therapy, and assessing results.

We prefer candidates with:

• At least six months to one year of clinical experience in a healthcare or mental health setting, ideally with exposure to hospice or palliative care.
• Specific training or experience in addressing the psychosocial needs of patients at the end-of-life stage, focusing on both emotional support and practical resource navigation.

Clear Sky offers a competitive salary range of USD 85,000 to 100,000 annually, commensurate with experience.
