Registered Nurse RN / Hospice

Coverage Territory: Ft. Wayne and surrounding areas

Schedule: M-F; 8a-5pm 

 Salary:  $82,000 - $90,000 / year + extra per visit pay for visits outside of normal shift

What You'll Do

As an RN Case Manager, you'll be the clinical anchor for your patients and their families — coordinating care, guiding the interdisciplinary team, and showing up when it matters most. Your day-to-day includes:

  • Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
  • Building and adjusting personalized care plans in collaboration with physicians and care teams
  • Administering medications and treatments per physician orders
