Job description

POSITION SUMMARY We are pleased to offer the opportunity to work as a Case Manager within the DuPage County Department of Community Services. Case Managers provide comprehensive case management and/or service coordination in a variety of areas. The current Case Manager opening is in the following programs/assignment area: Community Care Program (CCP)- Completes a comprehensive assessment with clients 60 and over to identify areas of need.

Case Managers in this program assist qualified clients with setting up services, such as CCP and/or Home Delivered Meals. They will also complete Medicaid application for clients who meet those eligibility requirements. CCP Case Managers make a variety of referrals to community partners which can include: caregiver support, counseling, energy assistance, and more.

The goal of this program is to connect eligible Seniors to various grant funding sources and resources that ensure they can remain stable and safe in the home. These positions are dependent on available grant funding. These positions are eligible for our complete benefit package including medical, dental and vision insurance, flexible spending accounts, life insurance, pension, deferred compensation plan, tuition reimbursement, pre-paid legal and credit union.

or a B.S. Degree in Social Science, Human Services, Social Work, or a related field or be a RN or LPN One (1) year of experience in assessment or service provision Certain programs may allow years of experience to replace educational requirements Must possess and maintain in good standing, a valid Illinois Driver's License A pre-employment background check is required.
