Expect flexible work hours from Monday to Friday, 9 AM - 5 PM, aimed at accommodating both client needs and your personal work-life balance.

• Provide targeted emotional support to patients enduring behavioral health crises, offering guidance and reassurance through challenging times.
• Manage case referrals by conducting thorough assessments to identify resource needs, monitor case progress, and facilitate appropriate closures.
• Help patients and families set and achieve their recovery goals through personalized psychoeducation and connections to community resources.
• Evaluate social determinants of health impacting care access, such as housing, income levels, food insecurity, and transportation accessibility.
• Facilitate access to support groups and community networks to enhance collective mental wellness among clients.
• Collaborate with an interdisciplinary team to align treatment strategies and goals tailored to each patient’s needs.
• Participate actively in ongoing consultations with team members to review patient progress and adjust care plans as necessary.

• A Master’s degree in Social Work (MSW) is required.
• Active and valid LCSW license to practice in California is mandatory.

• Strong understanding of community resources and social services, which is crucial for successful case management.
• Excellent organizational skills and the ability to manage multiple tasks simultaneously in a dynamic environment.

• Experience working with child and adolescent populations regarding mental health concerns.
• Exposure to acute behavioral health needs, including suicidal ideation and severe persistent mental illness.
• Demonstrated ability to work within a team framework to develop and implement comprehensive care plans.
