Southern IL Case Coordination Services is dedicated to providing conflict-free, person-centered case management to individuals with developmental disabilities. Key Responsibilities:

Develop comprehensive Personal Plans tailored to each individual's needs and goals.

Collaborate with service providers to ensure the individual's health, safety, welfare, and overall well-being are maintained at desired levels.

Assist individuals and families in understanding available services and making informed choices.

Coordinate and monitor service delivery to ensure satisfaction and effectiveness.

Work independently from home, managing your schedule to meet the needs of clients and service providers.

Maintain accurate documentation and records of case activities and progress.

Participate in ongoing training and professional development to stay current with best practices in case management.


Skills and Qualifications:

Bachelor’s Degree in a relevant field such as social work, human services, psychology, or related discipline.

Minimum of 1 year experience working with individuals with developmental disabilities or in a related human services role.

Strong interpersonal and communication skills to effectively collaborate with clients, families, and service providers.

Ability to work independently, manage time effectively, and prioritize responsibilities.

Knowledge of community resources, service options, and legal rights related to individuals with developmental disabilities.
