Job description

Find your calling at Mercy!Under the direction of the primary care operations team, the Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel. Duties and responsibilities will be performed in a manner consistent with our mission, values, and Mercy Service Standards.Position Details:Mercy Clinic Primary Care 621 S. New Ballas Road, Medical Tower A,Suite 399
St. Louis,MO63141

Schedule: Monday - Friday, 0800-1630

No nights, weekends, or holidays!

**Must complete a minimum of three months of on-site training prior to transitioning to a hybrid work-from-home schedule. After training, this role follows a 50/50 hybrid model, with full-time (1.0 FTE) co-workers expected to work on-site five days per pay period. Must also be available to attend monthly on-site clinic meetings.**

Our Primary Care Nurse is a newly created role for the clinics. In this position the nurse will work closely with the Providers and manage triage calls/messages for the clinic. They will have the option to move hybrid after training is complete.

LPN Qualifications:

Education: Diploma in Practical Nursing.

Licensure: Active LPN license.

Experience: 2-3 years of experience preferred.

RN Qualifications:

Education: Associate of Applied Science in Registered Nursing, Graduation from registered nursing program, bachelor's degree in nursing (BSN), OR 4 years of commensurate experience in lieu of the bachelor's degree.

Licensure: Active RN license.

Experience: 2-3 years of experience preferred.

Highly Preferred Skills:

Proficiency with Epic EMR system

Timely and thorough patient triage skills

Computer competency (most of this position will be computer work)

Excellent time management and prioritization

Adaptability in a new and changing environment
