Information & Referral Coordinator
The Independent Living division of The League is seeking a dedicated, hard-working and self-motivated Information & Referral Coordinator to provide information and referral to consumers, staff, businesses, organizations and the community at large. The ideal candidate should have a strong work-ethic, excellent customer service and problem-solving skills, and be motivated to serve our community . Prefer a qualified person with a disability .
Essential Duties/Skills/Responsibilities/Qualifications
• Conducts initial assessment to determine consumer 's individual needs to determine if The League provides services that can assist the consumer.
• Connect individuals with disabilities to community resources beyond The League's services.
• Conducts research to assist other League staff members in their efforts to provide l&R services.
• Provides resource information regarding housing, healthcare, transportation, and/or adaptive equipment.
• Educates consumers on civil rights under the Americans with Disabilities Act (ADA).
• Facilitates public presentations to promote The League's mission and disability issues
• Participates in individual and systems advocacy.
• Provides individual peer support.
• Coordinates the Christmas Bureau assistance program for The League's consumers.
• Represents the League at community health fairs, conducting public education, awareness and outreach activities.
• Administers the League's consumer data collection system, including new staff profile setup, data review, retrieval, reporting, staff training, consumer survey entry, and procedures management.
• Assist in consumer information file audits.
• Bachelor's degree or an equivalent combination of education and experience. Prefer a qualified person with a disability.
