Lifetime Assistance - Self-Hire Direct Support Professional

Position Overview:
  • Job Title: Self-Hire Direct Support Professional
  • Location: Varies by individual receiving services - Greater Rochester Area
  • Department: Community Services
  • Reports To: Fiscal Intermediary
  • Employment Type: Part-Time, various schedules available
  • Starting Wage: Based on individual budget and plan

Your Core Responsibilities:
  • Support individuals in attending and following up on medical appointments.
  • Educate on identifying abuse and accessing resources for safety.
  • Assist with securing alternative housing when needed.
  • Help maintain a safe and healthy home environment.
  • Facilitate participation in classes aligned with individual interests (e.g., nutrition, budgeting, music, art).
  • Complete and submit accurate daily and monthly documentation, including Medicaid billing and EVV data.
  • Attend required meetings and trainings.
  • Transport individuals as needed, following agency vehicle and mileage policies.
  • Comply with OPWDD and NYS Justice Center reporting regulations.

What You Bring:
  • High School Diploma or equivalent, preferred.
  • Ability to follow individualized Staff Action Plans and safeguard protocols.
  • Strong communication skills and ability to work collaboratively with individuals and their families.
  • Willingness to learn and adapt based on person-centered training.
  • Ability to lift up to 35 pounds and meet transportation requirements if applicable.
  • Commitment to accurate documentation and Medicaid compliance.
