Job description

Purpose of Position- Responsible for the day-to-day oversight and quality of 24/7, 365 day Community Partnerships program, including independent living, residential supports, provider-managed residential supports, and non-residential services operating under Imagine!'s Home Care Agency license. Provides supervision, compliance oversight, quality assurance, process development, stakeholder partnership management, and direct support. Provides monitoring and support to community partners staff and contractors to ensure health, safety, wellbeing, and a high quality of life of individuals served. Maintains professional relationships with all individuals receiving services, staff, contractors, and stakeholders. 

Essential Duties/Responsibilities

  • Staff supervision and support
    • Provides supervision, support, and accountability to Community Partnerships direct care staff, including recruitment, onboarding, performance management, coaching, professional development, and disciplinary action.
    • Supports and sustains a positive team culture that reflects Imagine!'s values and promotes person-centered, community-based supports.
    • Develops and trains staff through coaching, mentoring, and professional development while fostering a culture of accountability, respect, collaboration, and continuous learning.
  • Direct care and client support
    • Promotes and models person-centered practices, ensuring individuals have meaningful choice, community inclusion, and opportunities to achieve their personal goals.
    • Maintains a limited caseload and provides direct support as needed to ensure continuity of services, address complex needs,
    • Participates in the on-call rotation and provides leadership during urgent situations, critical incidents, and emergency response activities.
    • Coordinates services with case management agencies, healthcare providers, behavioral health professionals, guardians, and interdisciplinary team members to support successful outcomes for individuals served.
    • Assists with resource navigation among community partners, local government agencies, and family resource centers, acting as a support for staff to assist with case management for their caseload
  • Program support, evaluation and improvement
    • Oversees the daily operations of residential and non-residential services, including those provided under Imagine!'s Home Care Agency license.
    • Maintains current, up-to-date knowledge of all regulatory issues and changes and ensures residential program compliance with all standards and regulations, including Imagine! policies and procedures.
    • Monitors compliance with applicable regulations, licensing requirements, contractual obligations, and Imagine! policies and procedures for Community Partnerships services and Class B licensed programs.
    • Supports financial viability of Community Partnerships services through responsible staff scheduling and caseload assignments, monitoring service utilization, and ensuring accurate documentation and billing practices.
    • Promotes high-quality service delivery through ongoing evaluation of individual outcomes, staff performance, client and guardian satisfaction, and stakeholder feedback.
    • Leads programmatic quality assurance activities across residential and non-residential settings, including audits, home visits, fire drills, Human Rights Committee reporting and follow-up, HCA compliance activities, documentation review, and corrective action planning.
  • Community partnerships and stakeholder engagement
    • Cultivates and maintains strong, collaborative relationships with local community organizations, housing authorities, service providers, and advocacy groups.
    • Identifies and develops opportunities that increase community inclusion, access to resources, natural supports, housing stability, employment, and meaningful participation for individuals served.
    • Supports individuals with a variety of disabilities (IDD, brain injury, autism, physical disabilities, etc.) coming from a wide array of backgrounds and program staff to identify and use community resources and connections to maintain independence.

 

Job Qualifications

Knowledge, Skill, and Ability:

  • General knowledge of long-term care services and supports system in Colorado.
  • Knowledge of rules and regulations governing the operation of Individual residential supports and services (IRSS) and HCA home care settings.
  • Demonstrated skill in working with adults with intellectual and developmental disabilities.
  • Effective written and verbal communication skills with individuals and groups at all professional levels.
  • Effective and creative problem solver.
  • Ability to work independently and prioritize tasks/goals for self and others.
  • Ability to work effectively with various technology including word processing software, internet based databases, assistive technology, etc.
  • Possession of a valid driver's license and ability to meet Imagine! driving requirements.
  • This position requires regular use of a personal vehicle with liability insurance that meets Imagine! standards.
  • This position requires regular use of a personal mobile device such as a smartphone or tablet.

            Training/Education:

  • High School diploma or GED required.
  • Bachelor's degree in human services or related field preferred.   

            Experience:

  • Three years of related experience OR associates degree plus one year related experience.  
  • Supervisory experience preferred.
  • Must be at least 19 years old and have had a valid driver's license for at least 3 years.

            Working Environment/Physical Activities

  • Ability to lift 50 pounds.
  • Effective with shifting roles, responsibilities, and expectations in a changing environment. 
  • Regular travel throughout the Front Range area.
  • Willingness and ability to respond to emergency situations as they arise.
