Job description

We are actively seeking an experienced Director of Nursing (DON) for a Medicare-certified Home Health Agency serving Miami-Dade and Monroe Counties.

This is a high-impact HYBRID RN LEADERSHIP ROLE responsible for:

✔ Clinical oversight

✔ OASIS management and review

✔ QA / Performance Improvement

✔ Medicare and CHAP compliance

✔ Clinical supervision and operational coordination

✔ Support census growth and strengthen referral relationships

Ideal for an experienced Home Health DON, Home Health RN, Clinical Supervisor, or Clinical Manager seeking a flexible leadership role within a growing organization offering autonomy, flexibility, and long-term growth opportunity.

KEY RESPONSIBILITIES

HOME HEALTH CLINICAL LEADERSHIP

• Oversee timely admission and reassessment of patient needs

• Provide clinical oversight of skilled nursing services in the home setting

• Supervise RNs, LPNs, and contracted clinicians

• Complete or oversee OASIS assessments (SOC, ROC, Recertification, Discharge)

• Review and approve Plan of Care (CMS Form 485) and physician orders

• Ensure timely submission of NOA and OASIS validation reports

• Provide ongoing clinical guidance and support to field staff

• Conduct supervisory visits and clinical assessments as needed

• Ensure compliance with Medicare Home Health standards

QA / PERFORMANCE IMPROVEMENT (CHAP ACCREDITED)

• Oversee QAPI program and performance improvement initiatives

• Conduct chart audits and clinical documentation reviews

• Ensure compliance with Medicare Conditions of Participation (CoPs)

• Support CHAP, AHCA, CMS survey readiness and regulatory compliance

• Maintain accurate and timely clinical documentation

CLINICAL SUPERVISION & STAFF OVERSIGHT

• Coordinate staffing based on census needs

• Support onboarding, orientation, and clinical training

• Provide mentorship and performance feedback to clinical staff

• Approve patient assignments and supervise field clinicians

MEDICARE / REGULATORY COMPLIANCE

• Ensure compliance with Medicare-certified Home Health regulations

• Maintain adherence to CHAP and Florida AHCA standards

• Support billing and documentation integrity

• Maintain survey readiness and policy compliance

CENSUS GROWTH & REFERRAL DEVELOPMENT

• Collaborate with leadership on census growth initiatives

• Support clinical excellence to strengthen referral relationships

• Assist in building scalable clinical workflows and systems

HYBRID WORK STRUCTURE

• Majority of administrative duties performed remotely

• Occasional field visits for supervision, oversight, and direct patient care as needed

• Flexible scheduling based on agency and patient needs

• High autonomy leadership role

Qualifications

  • Active Florida RN license (required)
  • Home health DON or clinical leadership experience
  • Strong knowledge of Medicare CoPs and OASIS
  • Experience with CHAP/ACHC/state survey readiness
  • Background in QAPI, audits, and quality improvement
  • Leadership, mentoring, and staff supervision skills
  • Strong communication and organizational abilities
  • Proven experience in Home Health Care, with at least 1 years of experience.
  • Bilingual in English and Spanish.
