Job description


Coordinates and manages patient triage and care activities through telephone, electronic messaging, and limited in-office support, serving as a clinical resource for patients and providers within established nursing guidelines. Responsibilities include symptom assessment, medication management, prior authorization support, care coordination, patient education, review of diagnostic results, collaboration with providers, and disposition of patient concerns. This hybrid position supports a high-volume ambulatory practice through a combination of telework and on-site coverage, ensuring timely patient access and continuity of care.

ESSENTIAL FUNCTIONS

  • Provides compassionate and high-quality nursing care by planning, organizing, and directing the nursing functions of patients on the unit.
  •  Initiates and implements patient care plan.
  • Makes nursing assignments appropriate to the skill level of employees.
  • Maintains evidence-based standards of patient care.

Education and Experience

  • Bachelor's Degree, associate's degree, or diploma in Nursing from an accredited institution.
  • Current Maryland RN licensure or eligibility to obtain Maryland licensure.
  • Two to three years of clinical experience, preferably in ambulatory care, primary care, or telephone triage.
  • Strong assessment, communication, customer service, and care coordination skills.
  • CPR certification required.
