The Pharmacy Fulfillment Technician/ Care Management Support Assistant 2
works as a liaison between case managers, doctors, nurses and patients as they transition on service with One Infusion Pharmacy. Fulfillment Technicians are responsible for coordinating care with external and internal departments to ensure accuracy and completeness of all pertinent clinical, billing and demographic information in accordance with the policies and procedures of One Infusion Pharmacy, state regulations, and other regulatory entities such as Accreditation Counsel for Home Care (ACHC). The Fulfillment Technician works under the direct supervision of a licensed pharmacists and assists with coordination and delivery of products and services to One Infusion Pharmacy's patients. Patient registration, onboarding and coordination of care will comprise at least 50% of the daily tasks for the referral coordinator
Performs the following tasks in a correct and timely fashion under the supervision of a pharmacist:
  • Coordinates the onboarding of new patients to One Infusion Pharmacy by communicating with case managers, nursing and the caregivers to get the appropriate information

  • Communicates with patients and/or caregivers and internal dispatch team to schedule deliveries in a timely manner and optimal manner

  • Gathers all necessary medical documentation from providers, nurses and patients to aid the accurate dispensing of medications by the pharmacist

  • Accurately inputs pertinent clinical and demographic information into the computer system in order to build an electronic medical record

  • Proactively grows and maintains relationships with referral sources and contracted providers serving as a key point of contact

  • Adheres to appropriate safety and quality assurance practices.

  • Complies with state and federal laws.

  • Assists with product purchasing, inventory, and equipment management as needed.

  • Orders the correct drug products from the wholesale company if requested.

  • Knowledgeable of various vendors and where to get different supplies, medications and clean room needs. Verifies the accuracy of product orders received.

  • Checks prices against contract and in the schedule of charges. Complies with the One Infusion Pharmacy's formulary.

  • Speaks knowledgeably of the scope of services that are available through One Infusion Pharmacy. Understands basic drug terminology and therapies/services provided. Understands other areas of One Infusion Pharmacy and what scope of service the company has to offer.

  • Maintains confidentiality of patient and proprietary information.

  • Observes legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary One Infusion Pharmacy information.

  • Understands and complies with HIPPA.

  • Participates in the process for assessing, managing, and reporting medication-related incidents and equipment error and/or failures.

  • Accurately completes incident forms and forwards them to the supervising pharmacist and/or immediate supervisor.

  • Assists with the training and supervision of new referral coordinators.

  • In all activities, complies with accreditation, legal, regulatory, and safety requirements.

  • Participates in on-call responsibilities of the pharmacy department if required.

  • May work on advanced projects and committees, participate in performance improvement, complete and monitor quality control, and assist the purchasing coordinator.

  • Performs other duties as assigned by direct supervisor, Sr. Pharmacy Director and/or Clinical Pharmacists

Basic Qualifications:
  • Pharmacy Technician License Required

  • Basic level skill and competency with Microsoft Word, Excel, and Power point

  • Ability to comprehend medical information and transcribe into an electronic patient chart

  • Demonstrates initiative and can work on tasks autonomously performing all duties with excellence.

  • Strong organizational and planning skills as well as the ability to multi-task

  • Excellent phone etiquette and de-escalation abilities

  • Demonstrates basic level computer competency and has experience with one or more patient care registration or EMR systems such as Navihealth, Allscripts etc

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
