We are currently recruiting for a Weekend Hospice Nurse (RN or LPN) in Tucson to join our hospice's after hours nursing team. As an Weekend Hospice Nurse you are the focal point of care management across our interdisciplinary team for our patients after hours.

Our after hours program provides a number of exceptional support systems for both our patients, and our on call staff. As an after hours nurse at Agape, you are never alone. You work with other members of the after hours team in coordination of visits to patients who are in need of care outside our normal business hours.

Job Title/Position: Weekend Hospice Nurse

Reports To: Director of Clinical Services

Job Description Summary

Responsible for ensuring proper coordination of the after hours team during after hours. Ensures the after hours care needs of our patients are met in a timely manner, and that proper coordination back to the regular hours team is conducted in an effective manner.

Responsible for managing all aspects of the patient intake process after hours, including managing the members of the intake team, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.

Essential Job Functions/Responsibilities

  • Every weekend availability preferred, but rotating weekends will be considered based on the candidate

  • Conducts patient visits and gathers information for RN Case Manager.

  • Performs treatments, including wound care, as directed by the RN Case Manager.

  • Utilizes data gathered to determine nursing diagnoses and incorporate these diagnosis into plan of care under the direction of the RN Case Manager

  • Follows IDT Plan of care when providing hospice services to the patient.

  • Assist the RN Case Manager in educating the patient and/or their family as needed on medication and care.

  • Assists the RN Case Manager in counseling patient and/or family as related to healthcare needs.

  • Assists in identifying discharge-planning needs.

  • Reports changes in the patient’s physical condition or care needs to PCP.

  • Obtains and records physician’s orders as needed.

  • Orders medication, DME and other supplies to be delivered to patient as needed.

  • Reports any incidents of non-compliance to Chief Compliance Officer.

  • Follows all Agape Hospice & Palliative Care policies and procedures.

  • Participates in quality assessment performance improvement activities.

  • Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Agape Hospice & Palliative Care

Position Qualifications

  • Valid Arizona Driver’s License.

  • Automobile in good working order.

  • Proof of current automobile insurance.

  • RN / LPN with current licensure to practice professional nursing in the state.

  • Able to read, write and speak English fluently.

  • Thorough working knowledge of core nursing skills and is able to apply such knowledge of practices, techniques, procedures, and principles to ensure the highest quality of nursing care possible is provided to patients.

  • Ability to assess patient’s needs and direct care to meet those needs

  • Ability to deal tactfully with customers and the community.

  • Demonstrates good communications, negotiation, and public relations skills.

  • Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities
