Description

Human Touch Home Health is hiring a licensed Physical Therapist to provide home health services throughout Northern Northampton County, Pennsylvania, including Pen Argyl, Wind Gap, Bangor, and surrounding communities.

If you're looking for a flexible PRN opportunity where you can create your own schedule and provide one-on-one care in patients' homes, we'd love to hear from you.

What You'll Do
  • Complete physical therapy evaluations, reassessments, and discharge planning
  • Develop individualized treatment plans and provide skilled therapy services in patients' homes
  • Help patients improve strength, balance, mobility, and independence
  • Coordinate care with physicians and the interdisciplinary team
  • Complete documentation accurately and on time
