Job Duties
A. Performance Management (for employees who supervise others)
โ€ข Serves in a managerial/supervisory capacity to conduct performance management activities and ensure onboarding/offboarding and training for staff
โ€ข Establishes and reviews work assignments and priorities and implements performance improvement strategies and/or problem resolution for related issues, in conjunction with program management and Human Resource staff
โ€ข Ensures relevant training and workplace safety for staff
โ€ข Conducts required performance monitoring and appraisals, establishes clear performance expectations, addresses deficiencies in a timely manner, and documents underperformance in accordance with state guidelines
โ€ข Supports employee growth through regular feedback, coaching, and professional development opportunities
* If Employee Level Indicator is checked Supervisor or Manager, Performance Management Duties are applicable and must be included on the PD in PageUp.
B. Program Management
โ€ข Provides administrative and operational program support
โ€ข Maintains all required documents, files, and records in compliance with applicable requirements and policies and procedures
โ€ข Enters all program related data into database; ensures accuracy and completeness
โ€ข Analyzes information to make recommendations for improvement
โ€ข Contacts service providers (medical providers, laboratories, etc.) to provide and/or obtain information related to program services
โ€ข Completes and submits reports, records, records, and other required forms within established timeframes
โ€ข Provides information on community services available to clients (medical facilities, community based organizations, social services programs, etc.)
C. Case Management (if applicable)
โ€ข Provides effective case management services, including but not limited to:
o Contacts patients newly diagnosed; offers care and medical case management, according to the standards and guidelines
o Motivates patients to attend medical appointments and remain adherent to medication regimen
o Through the use of motivational interviewing techniques, identifies barriers that prevent the patient from attending appointments
o Links clients to additional services to reduce the barriers identified
o Identifies intervention and prevention opportunities
D. Grant and/or Fiscal Support
โ€ข Maintains knowledge of grant regulations and requirements as applicable and ensures that program activities and initiatives are in compliance
โ€ข Provides fiscal administrative duties and supports effective resource allocation
โ€ข Works to strengthen relationships with community partners to ensure seamless processes and efficient identification of areas of concern

This is a restricted position, which is solely funded by the Title V-MCH Maternal Child Health for a one year, period ending 06/30/2027. The availability of funding is scheduled for review annually and periodically thereafter. Continued employment is contingent on the continued availability of funds.

Minimum Qualifications
โ€ข Knowledge and/or experience with administration, planning, service delivery, and evaluation of principles and practices that support reporting, surveillance, and quality assurance for public health initiatives
โ€ข Knowledge and experience conducting studies and research, providing technical training, developing and communicating information, ensuring compliance with policy and regulations, and maintaining documentation
โ€ข Knowledge and/or experience with data collection and analysis, ensuring data accuracy and integrity, and producing reports
โ€ข Experience identifying, analyzing, and providing recommendations for problem resolution
โ€ข Experience working as part of a team, responding to inquiries in a professional and timely manner, interpreting and applying policies and procedures, and preparing correspondence
โ€ข Demonstrated effective oral and written communication, organizational, time management, analytical, interpersonal, and decision-making skills
Additional Considerations

โ€ข Experience with grant writing and/or grant acquisition and management
โ€ข Experience providing leadership and/or supervision of staff and teams
