Join our CCBHC - IA team at The Robert Young Center as a Care Coordinator, supporting adults, children, and families by assessing needs, developing person-centered care plans, and connecting clients with behavioral health, substance use, medical, and community resources. You'll coordinate services, assist with transitions from inpatient and residential care into the community, provide crisis support, and collaborate with community partners to help clients achieve positive outcomes.

  • Coordinates treatment plans and provides ongoing care management for clients with behavioral health needs.
  • Conducts triage and referrals, including responding to urgent and crisis situations with appropriate intervention and recommendations.
  • Connects clients with behavioral health services, community resources, state benefits, and support related to Social Determinants of Health (SDOH).
  • Collaborates with providers, social workers, nursing staff, families, and community agencies to coordinate comprehensive care.
  • Maintains accurate and timely clinical documentation, including client needs, changes in condition, and provider consultations.
  • Arranges transportation or transports clients to appointments and needed services.
  • Builds relationships with community agencies and participates in outreach events to educate the community about available RYC/CCBHC services.

Qualifications
Education:

  • Current licensure or accreditation required when applicable to degree.
  • Intermediate licensure preferred.
