Hudson Valley LPN Float Pool
Rate: $37/hour
Shift: Flexible 
Schedule: 3 shifts a week including 1 weekend shift

Hudson Valley Float Pool is looking to add a Licensed Practical Nurse (LPN) to its nursing team. In this role, you'll provide high-quality nursing care, administer medications and treatments, and help ensure the safety, comfort, and well-being of residents. Looking for flexibility, variety, and the opportunity to make an impact across multiple skilled nursing and rehab communities? Join the Hudson Valley LPN Float Pool team supporting:

  • Taconic at Beacon – Beacon, NY
  • Taconic at Hopewell – Fishkill, NY
  • Taconic at Ulster – Highland, NY

RESPONSIBILITIES:

  • Provide nursing care in accordance with resident assessments, care plans, and facility policies
  • Administer medications, treatments, enteral feedings, and IV therapies within the LPN scope of practice
  • Monitor resident conditions and promptly communicate changes to providers, supervisors, and responsible parties
  • Accurately document care, treatments, observations, and medication administration
  • Supervise and support Nursing Assistants to ensure quality resident care
  • Maintain up-to-date resident care information and communicate pertinent information during shift changes
  • Follow infection control, safety, resident rights, and confidentiality standards
  • Participate in interdisciplinary care planning and quality improvement initiatives
  • Assist with admissions, transfers, and other nursing responsibilities as assigned

REQUIREMENTS:

  • High School Diploma or equivalent required; Associate's Degree preferred
  • Valid NYS Licensed Practical Nurse (LPN) License required
  • Current CPR Certification required
  • Experience in a skilled nursing facility or with the geriatric population preferred
  • Strong clinical, communication, and organizational skills
  • Knowledge of medication administration, infection control, and resident care practices
  • Ability to supervise nursing assistants and work effectively as part of an interdisciplinary team
  • Compassionate, resident-centered approach to care
