We are dedicated to assisting individuals and families in overcoming struggles, increasing independence, and building positive relationships. At Tueller Counseling, competent professionals work together in providing guidance, support, and skills training to clients to ensure recovery and resiliency. These services enable individuals and families to build and sustain productive, meaningful, and happy lives.

Purpose:

To work in collaboration with the client, family and professional team to establish and work towards meeting goals and objectives as outlined in the Individualized Treatment Plan. As well as, to provide collaboration that assesses, plans, links, coordinates, and monitors options and services that address a member's needs.


Job Responsibilities:

  • Working from a client's Individualized Treatment Plan, teach and assist assigned clients skills needed to increase independence and improve overall functioning.
  • Use evidence-based practices to assist clients in achieving these goals in community and home environments.
  • Assist in client treatment via meetings, collaboration, assessments, etc.
  • Complete accurate progress notes documenting all Skills Building and Case Management interventions.
  • Write assigned client's Individualized Treatment Plans and annual renewals.
  • Help the member to learn about, gain, and maintain access to services and providers.
  • Coordinate care by; collecting and compiling information to support assessment activities, referral and coordination to arrange for services and related activities, and follow up on coordinating care to ensure services are provided and member's needs are adequately addressed.
  • Develop a Case Management service plan in conjunction with the member and the member's treatment team.
  • Ensure to Case Management service plan includes identification of member's strengths, specific/measurable goals for identified needs, and activities that will support the member in meeting their individual Case Management goals.
  • Comply with any Insurance Carrier requirements to treat.
  • Other tasks as assigned.
