Direct Supervisor: Housing and Supportive Services Department Director
Status: Full-time (40 hours), exempt, with benefits
Position Summary:
This position is responsible for providing case management and other supportive services to transitional age youth (TAY, ages 18-24) who are homeless, at-risk of homelessness, or living in supportive housing programs, to assist them in achieving and maintaining safe and stable housing and increasing overall self-sufficiency. Services include assessment, service/treatment planning, linkage, advocacy, coordination, and monitoring to assist clients in gaining access to needed health and dental services, financial assistance, housing, employment, education, social services, and other services and natural supports developed through a client-centered service planning process.
Housing case managers may work across various housing programs, including Transitional Living Program (TLP), Emergency Shelter, Rapid Re-Housing (RRH), Supportive and Permanent Supportive Housing, Family Unification Program (FUP) Vouchers, Fostering Youth to Independence Program (FYI) Vouchers, Housing Choice Vouchers, etc.
Education Qualifications
- Bachelor’s degree in social work or equivalent combination of training amp; experience
- 2+ years of experience working with homeless, runaway, or other youth in crisis.
- Experience facilitating groups and knowledge of group dynamics.
- Comfort with outreach work and meeting clients in their own homes or in the community.
- Ability to effectively engage youth with diverse identities, including intersecting identities of race, ethnicity, socioeconomic background and sexual orientation/gender identity and expression.
- Ability to effectively manage crisis situations.
- Ability to collaborate effectively with public systems and other providers.
- Commitment to best practices including Positive Youth Development, Harm-Reduction, Housing First and Trauma Informed approaches.
- Work requires effective written and verbal communication and interpersonal skills; ability to produce quality work within tight timeframes and simultaneously manage several projects; ability to participate in and facilitate group meetings.
- Possess strong work ethic and ability to take initiative. Demonstrate a high level of personal and professional integrity and trustworthiness.
- Successful completion of pre-employment and annual background check; valid driver’s license, current insurance, negative TB test, and access to reliable transportation.
Job Responsibilities and Duties
Case Management:
- Provide case management services to homeless and at-risk youth and their families, including resource information, linkage, coordination, advocacy, and other assistance.
- Maintain an active caseload of 15 – 20 clients at any given time.
- Conduct initial and on-going assessments of strengths and barriers to healthy functioning. Assess needs, make eligibility determinations, and facilitate referral or placement of clients seeking emergency, transitional, or independent housing assistance.
- Conduct comprehensive assessments for clients that will help the youth in identifying strengths and challenges and will assist in the development of a service plan.
- Provide independent living skills training to clients, including budgeting, nutrition, home care, leases, personal hygiene, healthy relationships, computer literacy and online safety, employment and education skills and support, and civic engagement.
- Utilize intake and assessment information to develop an individualized service plan in conjunction with the youth that identifies goals, action steps, services needed, and timeframes with an outcome primary outcome of safe and stable housing.
- Assist and advocate for clients in obtaining important documents for youth that may include, but is not limited to state id’s, driver’s licenses, birth certificate and social security card.
- Assist clients in locating, obtaining, and maintaining safe and stable housing.
- Assist and advocate for clients in accessing and utilizing education and employment services and support.
- Assist and advocate for clients in applying for services for which they may qualify, including but not limited to SNAP benefits, Medicaid, Housing Choice Vouchers (HCV), SSI/SSA.
- Assist and advocate for clients in obtaining needed physical health, mental health, and substance abuse services.
- Provide transportation assistance directly or indirectly to clients to facilitate their achievement of service plan goals.
- Provide access to technology resources to facilitate youth achieving their service plan goals (e.g. computers and internet access)
- Assist clients in identifying and accessing opportunities for recreation and well-being pursuits.
- Regular monitoring, updating, and evaluation of individual service plans in conjunction with clients.
- Facilitate referrals and linkages to Ozone House and other community services and resources. Monitor follow-through and/or success of these connections.
- Complete documentation for each case assigned including assessments, service plans, progress notes, review forms, discharge summaries, and related forms in accordance with funding requirements, agency standards, and timeframes.
