Overview:
Seeking an experienced Nurse Case Manager II to support members enrolled in Medicare and Medicaid. This 100% remote role focuses on improving health outcomes through comprehensive care management and collaboration with members, providers, and community organizations.
Responsibilities:
Conduct thorough assessments of members’ health and social needs.
Develop and implement care plans using internal and external resources.
Apply regulatory guidelines and company policies to ensure accurate benefits administration.
Advocate for members to achieve optimal, cost-effective outcomes.
Collaborate effectively with remote teams using virtual tools.
Qualifications:
Required: RN, LCSW, or LCPC with active Illinois license.
3–5 years clinical experience; 2–3 years care management or discharge planning preferred.
Strong analytical, problem-solving, and communication skills.
Proficient in MS Office and navigating multiple systems.
Self-motivated, independent, and confident working remotely.
Schedule: Mon–Fri, 8 AM – 5 PM CST
