Nurse Case Manager II – Illinois (Remote)
Location: Illinois

Overview:
Seeking an experienced Nurse Case Manager II to support members enrolled in Medicare and Medicaid. This 100% remote role focuses on improving health outcomes through comprehensive care management and collaboration with members, providers, and community organizations.

Responsibilities:

  • Conduct thorough assessments of members’ health and social needs.

  • Develop and implement care plans using internal and external resources.

  • Apply regulatory guidelines and company policies to ensure accurate benefits administration.

  • Advocate for members to achieve optimal, cost-effective outcomes.

  • Collaborate effectively with remote teams using virtual tools.

Qualifications:

  • Required: RN, LCSW, or LCPC with active Illinois license.

  • 3–5 years clinical experience; 2–3 years care management or discharge planning preferred.

  • Strong analytical, problem-solving, and communication skills.

  • Proficient in MS Office and navigating multiple systems.

  • Self-motivated, independent, and confident working remotely.
