Job description

Position – Case Manager 1

Program – Hogan House

Location – Hartford

Salary - 22.00 hourly

Schedule – Full Time, 40 hours, 1st shift, Sunday- Thursday (7am-3pm)

Program Summary:

A residential step-down program where pregnant and/or parenting women are required to participate in substance use community treatment while living in a safe and structured environment. Clients will meet with their Case Manager on a weekly basis and attend daily psychoeducational groups. The program is designed to support women, as they return to their communities as a healthy and productive parent in recovery.

Position Summary:

The Case Manager 1 is responsible for delivering case management services to the Women & Children Substance Use Treatment Facility. Duties include intake assessments, development and implementation of individual care plans, liaison with treatment providers and community resources, facilitation and documentation of client and group activities as well as daily psychoeducation groups in accordance with Women’s and Children’s curriculum and DMHAS core topics.

The Case Manager 1 is a member of the care team providing care and comprehensive substance use treatment services to women who have young children or are expecting a child, at our residential Women and Children Substance Abuse Treatment facilities.

Requirements:

  • Some post secondary education
  • 2 years relevant experience
  • Valid Connecticut Driver’s License
  • Regular and predictable attendance is required

Orientation:

If selected for employment with The Connection, newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
