Job description

We are seeking a reliable, compassionate Host Home/Companion Care Provider in The Woodlands, Conroe, or a nearby area to provide a stable and supportive home environment for an adult female participating in the Texas HCS program.


The individual is approximately 40 years old, relatively independent/high-functioning, and has a Level of Need (LON) 5. We are specifically seeking a Host Home setting.


Responsibilities

  • Provide a safe, comfortable, and supportive home environment
  • Assist with daily living needs as appropriate
  • Encourage independence, choice, and community involvement
  • Support appointments, activities, and community outings
  • Provide transportation as needed
  • Follow the individual's service plan and HCS requirements
  • Maintain required documentation
  • Communicate regularly with the individual's support team
  • Provide consistent supervision and support based on assessed needs
  • Preferred Qualifications
  • Current or previous Texas HCS Host Home/Companion Care experience
  • Reside in The Woodlands, Conroe, or within reasonable distance of The Woodlands
  • Have a suitable, safe, and well-maintained home
  • Valid driver's license and reliable transportation
  • Ability to successfully complete required background checks and training
  • CPR/First Aid certification or willingness to obtain
  • Dependable, patient, respectful, and committed to supporting individuals with intellectual/developmental disabilities
