Job description

Job Title: Social Worker - Hospital Discharge 
Location: Bromley 
Pay Rate: 32 (Depending on experience) 

Liquid Personnel is recruiting a Children's Social Worker to join its client's Hospital Discharge team, based in Bromley.
To provide high-quality, person-centred social work within a hospital discharge team, ensuring safe, timely, and effective transitions from hospital to home or appropriate care settings. The role focuses on promoting independence, safeguarding adults, and delivering integrated care in collaboration with health professionals.

What will your responsibilities be? 

  • Undertake timely, comprehensive assessments of adults in hospital, identifying care and support needs under the Care Act 2014.
  • Identify and respond to safeguarding concerns in line with statutory duties and local procedures.
  • Complete Mental Capacity Act assessments and make Best Interest decisions where required.
  • Work collaboratively with health professionals, ward staff, and community services to ensure integrated discharge planning.
  • Maintain accurate and timely records, ensuring compliance with legal and organisational standards.

Qualifications and Experience: 

To be successful in this role you must have.  

  • Social Work England registration 
  • Eligible to work in the UK 
  • Hold a full UK driving licence 
  • Post-qualified experience 
  • Degree level or equivalent in Social Work
