About the Position:
We are looking for skilled and compassionate RN Case Managers to join our dynamic team. This role offers a starting pay rate of $55.91 per hour and comprehensive benefits through the 1199 Union, including health, dental, vision, and retirement plans.
Key Responsibilities:
  • Conduct comprehensive assessments of patients' needs, including medical, psychological, and social factors.
  • Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary teams.
  • Monitor and evaluate patients' progress and make necessary adjustments to care plans.
  • Facilitate effective communication between patients, families, and healthcare providers.
  • Advocate for patients' needs and ensure they receive appropriate services and resources.
