Per Diem Hospice Admissions Nurse

As a Per Diem Hospice Admissions Nurse, you will be responsible for executing the hospice admission process in compliance with federal and state regulations. You will advocate for the patient and family to meet their needs that hospice can cover and refer them to other resources as appropriate.

Primary Responsibilities:

  • Demonstrates competency with potential patients, hospice eligibility and meeting with families in assessment of their needs
  • Maintains confidentiality and safety of patient/client clinical records
  • Completes assigned admissions and patient care orders in an effective and timely manner
  • Communicates effectively with physician(s), referral sources, patient, family, and hospice interdisciplinary team regarding the new admission of a patient. Records that may be relevant to determine hospice eligibility
  • Communicates effectively with operational, clinical and sales leadership regarding status of pending referrals

Required Qualifications:

  • Current unrestricted RN licensed in the state of operation
  • 2+ years of experience in a hospice, home health, or other healthcare service delivery system setting
  • Current CPR certification
  • Current Driver's License and vehicle insurance, and access to a dependable vehicle or public transportation

Preferred Qualifications:

  • Demonstrated fiscal responsibility and accountability
  • Proficient computer skills with knowledge of Microsoft Word and Excel

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The hourly pay for this role will range from $81,920 to $122,880 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
