Job description

Next Care Hospice is a fast growing hospice agency that provides compassionate care to our patients and their families during the end-of-life journey.

We are committed in providing continuous training to our nurses with growth opportunities across the clinical and administrative tracks.

We are currently in need of an experienced Per-Diem Hospice RN to give skilled nursing to hospice patients at home and senior facilities. One year minimum experience is required. The RN must have experience in hospice care and be able to bring each visit to completion with recording of the notes in the EMR system (Hospice MD)

Our hospice patients are located in Covina, Chino Hills, and surrounding areas. The RN is expected to conduct supervisory visits, recertifications and new admissions.

You will be responsible for the assessment, diagnosis, and treatment of the assigned patients.

The ideal candidate will be a responsible and well-trained professional able to give the best nursing care with little supervision. You will be compassionate and cool-tempered. You will be able to follow health and safety guidelines faithfully and consistently. You will initiate and coordinate the hospice care plan, supervise CHHAs and LVNs and provides skilled nursing hospice and palliative care services provided to our patients. The RN will work cooperatively with the attending physician, Hospice Medical Director, and other members of the interdisciplinary Hospice team. The RN regularly reviews and updates the plan of care, performs prescribed medical treatments (including pain management and symptom control), conducts assessments and evaluations and provides education and supportive care to patient and caregivers. Must be able to utilize computers and be comfortable with EMR (Hospice MD). Hospice MD training can be provided.

QUALIFICATIONS: Current RN license within the State. Hospice experience required Knowledge of EMR software
