Position Summary

As a Per Diem Hospice Registered Nurse (RN), you will provide skilled nursing care to patients with life-limiting illnesses throughout the East Bay. You will help patients remain comfortable while supporting families through every stage of the hospice journey.

This role requires excellent assessment skills, compassionate communication, strong clinical judgment, and the ability to work independently while collaborating with an interdisciplinary team.

Responsibilities
  • Perform comprehensive hospice nursing assessments.
  • Develop, implement, and evaluate individualized plans of care.
  • Provide skilled nursing visits in patients' homes, assisted living facilities, board and care homes, and skilled nursing facilities.
  • Assess and manage pain and symptoms using evidence-based hospice practices.
  • Administer medications, treatments, and interventions as ordered.
  • Educate patients and caregivers regarding disease progression, medications, symptom management, and end-of-life care.
  • Coordinate care with physicians, social workers, chaplains, home health aides, and the interdisciplinary team.
  • Communicate changes in patient condition promptly to the Hospice Director and attending physician.
  • Complete accurate and timely electronic documentation using WellSky (Kinnser).
  • Participate in Interdisciplinary Group (IDG) meetings.
  • Maintain compliance with Medicare Conditions of Participation, California regulations, and agency policies.
  • Participate in Quality Assurance and Performance Improvement (QAPI) activities as assigned.
Qualifications
  • Current California Registered Nurse (RN) license in good standing.
  • Minimum one (1) year of RN experience required.
  • Previous Hospice or Home Health experience strongly preferred.
  • Excellent assessment and critical thinking skills.
  • Compassionate, dependable, and patient-centered.
  • Strong communication and organizational abilities.
  • Ability to work independently while collaborating with an interdisciplinary team.
  • Current California driver's license.
  • Reliable transportation with proof of auto insurance.
  • Current CPR/BLS certification.
