Hospice Registered Nurse (RN) Position Summary

Safro Staffing is partnering with a hospice care agency seeking an experienced Hospice Registered Nurse (RN) to provide compassionate, patient-centered care to individuals and families. The Hospice RN will coordinate care, perform comprehensive assessments, develop and update care plans, collaborate with interdisciplinary team members, and support patients and families throughout the hospice journey.

Responsibilities
  • Coordinate hospice care services in collaboration with patients, families, physicians, and interdisciplinary team members
  • Conduct comprehensive nursing assessments and hospice evaluations
  • Develop, implement, and update individualized plans of care
  • Monitor patient condition and respond appropriately to changes in clinical status
  • Provide education and emotional support to patients and their families
  • Coordinate care with contracted providers and community resources as needed
  • Maintain accurate and timely clinical documentation in accordance with regulatory requirements
  • Communicate effectively with attending physicians and the Hospice Medical Director regarding patient needs and care planning
  • Participate in interdisciplinary team meetings, quality improvement initiatives, and staff development activities
  • Support end-of-life care planning and symptom management to ensure patient comfort and dignity
Qualifications
