Summary of Duties
Using own automobile, performs direct and indirect total nursing care of the Hospice patient and family in accordance with the St. Francis Hospice Standards for Nursing Practice in the home setting; instructs family in simple nursing procedures in accordance with the Hospice Interdisciplinary Plan of Care.
About the Role
The RN-Hospice Home Care delivers specialized care to patients and their families, helping them navigate end-of-life journeys with respect and peace in familiar surroundings. The RN-Hospice Home Care partners with the interdisciplinary team, including physicians, social workers, and spiritual care providers to create and implement individualized care plans tailored to meet each patient's unique needs. The RN-Hospice Home Care works directly in the patient's home to perform direct and indirect total nursing care for the Hospice patient and family in accordance with the St. Francis Hospice Standards for Nursing Practice in the home setting; instructing the family in simple nursing procedures in accordance with the Hospice Interdisciplinary Plan of Care
Essential Functions of the Job
  • Provides care and maintains awareness of total nursing needs of patients/families assigned to him/her and is responsible for seeing they are fulfilled.
  • Administers and supervises the patient plan of care for each hospice patient for whom he/she is responsible, according to Federal, JCAHO, and appropriate Standards of Nursing Practice as defined by the Hospice Program.
  • Maintains communication and coordinates with primary physician and/or on-call physician in regard to the patient's plan of care.
  • Arranges for medical equipment, delivery/pick up of medications and supplies in the home setting when appropriate.
  • Completes Home Health Aide Care Plan and provides instructions accordingly.
  • Coordinates nursing service with other disciplines including social services, pastoral/spiritual care, and volunteer services.
  • Participates in Interdisciplinary Team Conferences; presents and identifies problems for evaluation and recommendation and then helps to coordinate the plan of care which will best meet the patient/families' needs.
  • Responsible for initiating Bereavement Assessment Form on assigned patients.
  • Assists in providing an optimal safe physical and emotional environment for both patient and family.
  • Teaches and coaches the patient and family members regarding the nursing care needs and other related problems of the patient.
  • Adheres to and effectively communicates Hospice's philosophy, goals, and objectives to community health agencies and to the public.
  • Works together with hospice inpatient RN when a patient is transferred into the facility or into the home hospice setting regarding medications, supplies, and equipment.
Requirements
Educational Requirements
Registered Nurse, B.S.N. preferred. State of Hawaii RN license required.
Experience
Minimum two (2) years of previous experience. Acute/Oncology Nursing Experience preferred.
Skills Required
  • Dexterity in handling equipment and instruments.
  • Must have valid driver's license.
  • Personal automobile available for business use.
  • Good driving record, as evidenced by a current driver's abstract.
  • Ability to adapt to changing situations on short notice and with ease.
  • Effective interpersonal, communication skills.
  • Desire to continually upgrade self in knowledge, skills, and professional conduct.
  • Current CPR certification.
