  • Full Time Permanent employment
    <\/li>\n
  • Schedule: Day Shift; Mon\-Fri with weekend and holiday rotation
    <\/li>\n
  • Collaborates with the hospice interdisciplinary team to develop and implement a comprehensive plan of care based on patient, family, and team assessments.
    <\/li>\n
  • Completes functional assessments and initiates the plan of care to identify needs, manage symptoms, and establish patient\/family goals.
    <\/li>\n
  • Provides direct nursing care, supervises care staff and volunteers, and evaluates progress using strong clinical and communication skills.

    Hospice Registered Nurse Qualifications:<\/b>

    • Unrestricted RN license with the ability to practice in Wisconsin
