As a RN Case Manager You will:ย 

  • Assess/monitor physical, emotional, and psychological needs of patientsย 
  • Create hospice care plans that align with the patient's wishes and goalsย 
  • Direct nursing care: administering medications, treatments, and interventionsย 
  • Provide pain and symptom managementย ย 
  • Educate and support the patient's family and caregiversย 
  • Collaborate with interdisciplinary teamย 
  • Maintain accurate and timely documentationย ย 
  • Participate in on-call rotation as required by local branchย

Qualifications
    • Graduate of an accredited nursing school with current licensure to practice in state of operationย 
    • One year nursing experience required, 3+ years preferredย 
    • Experience in hospice or a similar setting preferredย 
    • Knowledge of the hospice philosophy of careย 
    • Commitment to clinical and documentation excellenceย 
    • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working orderย
