Location: Remote / Work From Home
Pay: $32.00 - $40.00 per hour
Schedule: Saturday & Sunday, 8:00 AM - 5:00 PM (18 hours per week) PST

About the Role

Puget Sound Home Health & Hospice is seeking an experienced and compassionate Weekend Hospice RN Triage Nurse to provide remote clinical support to hospice patients, families, and caregivers. This role is responsible for telephone triage, symptom management, care coordination, and crisis support during weekend hours, helping ensure patients receive timely, high-quality end-of-life care.

The ideal candidate is a confident clinician with hospice experience, strong assessment skills, and the ability to provide calm, professional guidance during challenging situations.

Key Responsibilities

  • Phone Triage:Answer incoming calls from patients, families, and caregivers with empathy and urgency.
  • Symptom Management:Assess pain and distress, providing clinical advice and medication adjustments under established protocols.
  • Care Coordination:Dispatch visiting nurses, social workers, aides, or chaplains for urgent in-person needs.
  • Documentation:Record all calls, interventions, process workflow and care plans accurately in the electronic medical record (EMR) system.
  • Crisis Support:Guide families through the steps and procedures when a patient passes away at home.

Schedule

  • Every Saturday and Sunday working in PST time zone
  • 8:00 AM - 5:00 PM
  • Part-Time (18 hours per week)
QualificationsRequired
  • Active, unrestricted Registered Nurse (RN) license.
  • Minimum of two (2) years of nursing experience.
  • Hospice triage experience required.
  • Must be able to work in Pacific Time Zone.
  • Experience with Homecare Homebase (HCHB).
  • Strong clinical assessment, triage, and critical thinking skills.
  • Excellent communication and active listening abilities.
  • Ability to work independently in a remote environment.
  • Reliable high-speed internet and dedicated HIPAA compliant home workspace.
