Job description

Summary

We are hiring a Hospice Admissions Registered Nurse (RN) for a full-time, permanent opportunity in Denton County, TX. This role is ideal for compassionate RNs who are passionate about guiding patients and families through the hospice admission process while ensuring a smooth transition into hospice care.

Location

  • Denton County, TX

Salary

$85,000 – $95,000 annually

Additional Compensation

  • Mileage Reimbursement: $0.65 per mile
  • Cell Phone Reimbursement: $60/month

Schedule

  • Full-Time
  • Monday–Friday, 8:00 AM – 5:00 PM
  • On-call rotation required
  • Travel throughout assigned territory

Key Responsibilities

  • Evaluate and assess patients to determine hospice eligibility.
  • Guide patients and families through the hospice admission process with compassion and professionalism.
  • Develop individualized plans of care for newly admitted patients.
  • Coordinate patient transitions with the interdisciplinary hospice team.
  • Maintain accurate and timely clinical documentation.
  • Provide skilled nursing care in accordance with hospice standards.
  • Participate in limited on-call responsibilities.
  • Support additional clinical and administrative duties as assigned.

Requirements

  • Active Texas Registered Nurse (RN) License.
  • Minimum 1 year of Hospice or Senior Care experience.
  • Strong knowledge of hospice philosophy, eligibility requirements, and end-of-life care.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage daily responsibilities.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Must be at least 21 years of age.
  • Successful background check and professional references.
