Heritage Hospice is excited to resume services in the Albuquerque community. Our dedicated team is hiring and ready to provide compassionate, highquality care for patients and families across the region.

We are seeking a compassionate and dedicated PRN Hospice CNA to join our interdisciplinary care team. In this role, you'll provide personal care, comfort, and supportive services to patients and families. This position offers flexible scheduling and the opportunity to make a meaningful impact while working alongside a deeply committed hospice team.

Key Responsibilities

  • Provide handson personal care to hospice patients, including bathing, grooming, dressing, repositioning, and ADL support
  • Assist with maintaining a safe, clean, and comfortable environment in the home or facility setting
  • Observe and report changes in the patient's condition to the RN Case Manager
  • Offer emotional support to patients and families with sensitivity and respect
  • Document care accurately and in accordance with regulatory and organizational standards
  • Participate in team communication to ensure coordinated, highquality care
  • Travel to patient homes, facilities, and communities within assigned coverage area

Qualifications

  • Current New Mexico CNA certification
  • Prefer at least six months direct patient care experience; hospice or home health experience preferred
  • Reliable transportation and valid driver's license
  • Ability to work independently in home settings with professionalism and compassion
  • Strong communication, timemanagement, and interpersonal skills
  • Comfort supporting patients and families during endoflife transitions
