Job description

Hospice Registered Nurse Case Manager

LilyCare Hospice is a community-based hospice serving patients and families across central New Mexico. We are hiring a full-time Hospice Registered Nurse Case Manager to join our clinical team and help shape hospice care delivery from the ground up.

Duties and Responsibilities

  • Serve as the Hospice Registered Nurse Case Manager for a caseload of hospice patients in private homes and facilities — complete assessments, individualized plans of care, and routine visits

  • Provide skilled pain and symptom management in coordination with the attending physician and Hospice Medical Director

  • Participate in IDG meetings and coordinate care across the interdisciplinary team, including hospice aides, social work, and chaplaincy

  • Educate and support patients, families, and caregivers, and supervise hospice aide care per the plan of care

  • Complete timely, accurate EMR documentation and participate in the RN Case Manager on-call rotation

Compensation and Schedule

The Hospice Registered Nurse Case Manager role pays $90,000 - $105,000/year based on experience. Full-time, Monday–Friday day schedule with a rotating on-call assignment. Territory is based out of Albuquerque, NM.
