Job description

Position Overview
Start of Care Physical Therapist Horizon Home Health & Hospice is seeking a part-time, non-exempt, per-visit Start of Care (SOC) Physical Therapist to join our home-health team in Caldwell, Idaho. This role is ideal for PTs who excel in first-visit evaluations, functional mobility assessments, and establishing a strong therapeutic foundation for each patient's episode of care.
As a SOC PT, you will complete comprehensive admission assessments, initiate individualized treatment plans, and collaborate closely with our interdisciplinary team to ensure timely, accurate, and patient-centered service initiation.

Key Responsibilities
SOC PT Responsibilities
  • Complete Start-of-Care physical therapy evaluations in patient homes.
  • Develop individualized treatment plans focused on functional mobility, safety, and independence.
  • Conduct comprehensive assessments including gait analysis, strength testing, balance evaluation, and home-safety review.
  • Establish strong patient rapport and set expectations for the home-health episode.
  • Collaborate with interdisciplinary team members to ensure timely initiation of services.
  • Provide therapeutic interventions as needed during SOC visits.
  • Recommend and educate on durable medical equipment and home-safety modifications.
  • Maintain accurate, timely documentation in accordance with regulatory and organizational standards.
  • Communicate effectively with physicians, caregivers, and internal team members.

Qualifications
PT Requirements
  • Current Idaho Physical Therapist license (or ability to obtain).
  • One year of PT experience preferred; home-health or SOC experience a plus.
  • Strong communication, assessment, and problem-solving skills.
  • Ability to work independently in a field-based environment.
  • Reliable transportation and valid driver's license.
