Reports To: Nurse Manager


Employee Status: Non Exempt


Under general supervision, but in line with established Primary Care & Hope Health Center policies and procedures, provide patient care; conduct physical exams; complete health history summary; document patients' progress; assist with patient care management. Advise patients on health maintenance and disease prevention and provide case management.


Essential Functions

  • Represent Primary Care & Hope Health Center to the patients, employees, and guest in a courteous and professional manner. Provide prompt, efficient and accurate service...
  • Prepares exam room for use and sterilizes equipment and supplies.
  • Escort patients to exam room determining chief complaint and pertinent history.
  • Take and record vital signs, record various results (height, weight, vision, hearing, B/P, urinalysis, glucometer check, pregnancy test, etc) into the E-Clinical software.
  • Assists provider with procedures and patient care.
  • Perform, EKG, venipuncture, suture removal, breathing treatment, immunizations, eudiometry test, etc.
  • Call prescriptions into pharmacy.
  • Provide patient education as warranted communicating laboratory and diagnostic testing results by phone and mail.
  • Utilize eclinical software for patient records...
  • Triage calls for provider to include determining emergencies, identifying symptoms, using discretion for work in appointments and refilling routine prescriptions after reviewing medical chart.
  • Performs laboratory tests of minimal to moderate complexity, including completion of lab requisitions per computer and paper.
  • Maintain quality monitoring logs; i.e.; refrigerator, lab test, immunizations, lab inventory and stocking of supplies.
  • Assure all lab specimens have been picked up at day end...
  • Ensure the infection control standards and OSHA standards are followed and hazardous waste is properly disposed.
  • Contact drug companies as directed for medication samples.
  • Completes all required paperwork.
  • Plan and organize work load to maintain efficient patient operations in the health center.
  • Maintains a strong patient focus while working collaboratively within the teams multiple demands and patient needs.
  • Keep work area and patient rooms clean and free of clutter.
  • Treats all patient information confidentially and in accordance with established guidelines.
  • Direct and supervise less skilled nursing or health care personnel or supervise a particular unit.
  • Ensures proper cleanliness and sanitation of all patient rooms.
  • Improve the patient experience and health care processes and help assure the provision of high quality health care, as well as an ongoing process of performance improvement.
  • Manage time effectively and prioritizes completing tasks to meet deadlines.
  • Treats all clinic and employee information confidentially and in accordance with established guidelines.

Requirements

  • Must hold a current RN TN license.
  • Must maintain a CPR certification.
  • Knowledge of medical terminology.
  • Must be able to lift 30 pounds/walk and /or stand for 8 hour shits-bend or stoop/
  • Must be able to plan, prioritize, multi-task, organize and follow through while remaining highly energetic and focused.
  • Ability to work well under pressure.
  • Must communicate with patients/visitors in a friendly, positive, and professional manner.
  • Must have ability to work as a team player.
  • Must be accurate in all transactions...
