Home Health Scheduler/Caregiver (CMA Preferred)

Are you an organized, compassionate healthcare professional who enjoys problem-solving and making a difference in the lives of others? Western Home Communities is seeking a Home Health Scheduler/Caregiver (CMA Preferred) to join our growing Thrivera atHome team. This unique role combines scheduling coordination with hands-on client care. You'll be at the center of making care happen-bringing together the right caregivers, schedules, and client needs while supporting our team and helping those we serve maintain their independence and quality of life.

At Thrivera, you will join a supportive team that values collaboration, communication, and caring for one another while providing exceptional service to those we serve.

Location: In-person
Schedule: Full-time position with regular office hours (approximately 8-12 hours per week in the office), while also providing field support as needed for client needs, staffing coverage, and team vacations. This position includes participation in an on-call weekend and holiday rotation.

Requirements of Home Health Scheduler/Caregiver:

  • Current CMA certification is highly preferred.
  • Current CNA certification with the Iowa Nurse Aide Registry in good standing preferred; however, candidates with three or more years of caregiver experience may be considered with the expectation of obtaining CNA certification within the first six months of employment.
  • (Required) Must have a valid Driver's License
  • (Required) Must have reliable transportation and active vehicle insurance as you will be responsible for travel to the client's home
  • (Required) Must be willing and able to perform hands-on cares
  • (Preferred) Prior Scheduling experience
  • Excellent communication skills
  • Must be CPR certified or willing to obtain
  • Will be working independently, so must be a self-starter and able to move from one task to another

Responsibilities of Home Health Scheduler/Caregiver:

  • Coordinate scheduling for Thrivera atHome caregivers to ensure appropriate staffing coverage and timely support for client needs.
  • Communicate with caregivers, clients, families, and leadership regarding scheduling changes, staffing needs, and service concerns.
  • Provide hands-on client care and support, including personal care, activities of daily living, meal preparation, companionship, and maintaining a safe home environment.
  • Assist clients with daily activities including bathing, dressing, toileting, mobility, transfers, and other personal care needs.
  • Build positive relationships with clients and caregivers while promoting dignity, independence, and quality of life.
  • Respond promptly to client needs and communicate concerns appropriately to the Thrivera team.
  • Assist clients and families with questions regarding Thrivera atHome services and support options.
  • Maintain on-call availability during assigned non-business hours, including weekend and holiday rotations.
  • (CMA) Administer medications and complete required treatments within the scope of certification and under the direction of licensed nursing staff.
