Job description

Touching Hearts at Home – Fox Valley
Employment Type: [Full-Time / Part-Time]

Help Us Raise the Standard of Home Care

Touching Hearts at Home Fox Valley is looking for an experienced, compassionate Home Services Supervisor to help us build an exceptional care experience for both our clients and our caregivers.

This is not simply an office-management position.

Our Home Services Supervisor is a hands-on leader responsible for helping caregivers feel prepared, supported, and confident in the field while making sure our clients and their families receive safe, reliable, compassionate care.

You will play a central role in caregiver training and supervision, client satisfaction, quality assurance, scheduling support, and the day-to-day coordination of care.

If you believe great home care starts with great caregivers—and that caregivers perform their best when they have strong training, clear expectations, and a responsive supervisor behind them—we would like to meet you.

What You'll DoCaregiver Leadership & Development

  • Provide day-to-day supervision, direction, and support to Home Services Workers.
  • Help caregivers prepare for new client assignments and understand each client's service plan and expectations.
  • Provide initial training, orientation, continuing education, coaching, and ongoing supervision.
  • Ensure caregivers understand Touching Hearts at Home policies, procedures, professional boundaries, and standards of care.
  • Remain available to caregivers when questions or concerns arise regarding client care.
  • Identify opportunities for additional caregiver education, coaching, or competency development.
  • Support caregiver hiring, onboarding, orientation, training, and retention.
  • Address caregiver performance concerns promptly, professionally, and constructively.
  • Help create an environment where caregivers feel respected, supported, and accountable.

Client Care & Quality Assurance

  • Oversee the delivery of home services and help ensure each client's service plan is being followed safely and effectively.
  • Participate in client admissions and the transition of new clients into service.
  • Conduct required supervisory and quality-assurance visits in client homes.
  • Observe caregiver performance and provide appropriate coaching and follow-up.
  • Conduct client and family satisfaction surveys and check-ins.
  • Identify changes, concerns, or service gaps and coordinate appropriate follow-up.
  • Follow up on client or family grievances and work toward timely resolution.
  • Help maintain strong communication between clients, families, caregivers, and the office.
  • Coordinate client services with home health, hospice, rehabilitation, senior living, and other healthcare or community partners when appropriate.
  • Help ensure new client starts are organized, properly communicated, and staffed with an appropriate caregiver.

Scheduling & Daily Operations

  • Assist with caregiver scheduling and client assignments.
  • Help match caregivers and clients based on skills, personality, availability, location, and care needs.
  • Assist with call-offs, schedule changes, and coverage issues as needed.
  • Communicate changes quickly and professionally to caregivers, clients, and families.
  • Maintain strong telephone availability and responsiveness during working hours.
  • Help the agency maintain reliable service when unexpected situations arise.
  • Document supervisory visits, caregiver performance concerns, client issues, and required follow-up accurately and promptly.

Quality Improvement & Compliance

  • Participate in the agency's Quality Improvement Program, meetings, and improvement projects.
  • Help ensure caregiver training, supervision, documentation, and service delivery follow agency policies and applicable Illinois home-services requirements.
  • Monitor whether caregivers are providing services safely, effectively, and within their permitted scope.
  • Identify recurring service issues and recommend improvements to training, scheduling, communication, or agency procedures.
  • Perform additional responsibilities delegated by the Agency Manager.

Community & Referral Relationships

Home care works best when providers communicate and collaborate.

The Home Services Supervisor may also:

  • Develop and maintain relationships with referral partners and community organizations.
  • Represent Touching Hearts at Home professionally within the local senior-care community.
  • Communicate effectively with healthcare and senior-service professionals involved in shared clients.
  • Help make Touching Hearts at Home a trusted resource for seniors and families—even when our services may not be the right solution.

Required Qualifications

Candidates must:

  • Be at least 18 years of age.
  • Have appropriate experience or training in home care or closely related personal-care services.
  • Have completed training in the provision of personal-care services.
  • Possess strong verbal and written communication skills.
  • Be comfortable supervising, coaching, and providing constructive feedback to caregivers.
  • Be organized, dependable, responsive, and comfortable handling changing priorities.
  • Be comfortable working independently in client homes and a variety of residential environments.

In addition, candidates must meet at least one of the following Illinois Home Services Supervisor qualification pathways:

Option 1 – Direct Care Experience

Within the last five years, have at least two years of experience working in a:

  • Home services agency;
  • Home health setting;
  • Home nursing environment; or
  • Facility-based healthcare setting;

and have experience performing tasks permitted for Home Services Workers, including assistance with Activities of Daily Living (ADLs).

OR

Option 2 – Current Supervisor/Trainer Experience

Currently work in a supervisory or trainer position with a home services agency and have received appropriate training within the past year covering the tasks Home Services Workers are permitted to perform and the applicable standards of care.

What We're Looking For

The qualifications matter, but so does how you lead.

Our strongest candidate will be someone who:

  • Genuinely enjoys teaching and developing caregivers.
  • Can hold people accountable without making them feel unsupported.
  • Notices details before they become problems.
  • Is comfortable walking into a client's home and assessing whether care is actually working.
  • Communicates calmly with families during stressful situations.
  • Understands that caregiver-client matching matters.
  • Is willing to pick up the phone when a caregiver needs guidance.
  • Can balance empathy with clear professional boundaries.
  • Takes documentation and follow-through seriously.
  • Doesn't wait for problems to become emergencies.
  • Believes excellent caregiver support leads to excellent client care.

Experience as a CNA, home care supervisor, caregiver trainer, care coordinator, home health professional, assisted-living professional, or similar senior-care role may be especially valuable.

Physical & Environmental Requirements

This position may require the ability to:

  • Lift, push, pull, or carry up to 25 pounds.
  • Bend, twist, stoop, kneel, and reach.
  • Travel to and work within different client residences and care environments.
  • Communicate effectively in person and by telephone.
  • Read, write, speak, and understand English at an adult professional level.
  • Work in environments where there may periodically be pets, pet hair, dust, cleaning products, or similar household conditions.

Why Touching Hearts at Home Fox Valley?

We want to build an agency where exceptional caregivers want to work and where families know someone is paying attention.

That means taking caregiver training seriously, communicating with families, carefully matching caregivers and clients, responding when something changes, and supporting the people providing care instead of leaving them to solve every problem alone.

Our philosophy is simple:

Caregiver-centered. Client-focused.

We are building a team that wants to raise the standard of home care in the Fox Valley—and we are looking for a Home Services Supervisor who wants to help lead that effort.
